Biochem/physiol Actions
Albumin is synthesized exclusively by the liver parenchymal cells at a rate of 14 g/day. While it is the most abundant plasma protein (>50%), increased urine albumin concentrations indicate bleedings in the lower urinary tract (ureter, bladder) or infections of the renal pelvis. A small abnormal albumin excretion is known as Microalbuminuria and serves as an indicator for temporary overload of the glomerular filtration (fever, excessive sports) or chronic injury of the glomeruli (diabetes).

Biochem/physiol Actions
AMBP is synthesized by the liver. Approximately half of the circulating protein is complexed to IgA. The free form of AMBP is filtered by the glomerulus and reabsorbed by proximal tubule cells. AMBP has been found to be a sensitive biomarker for proximal tubular dysfunction even in the early phase of injury when no histologic damage is observable. In addition, urinary AMBP has been proposed to be a useful marker of tubular dysfunction even in low-gestational-age preterm infants, a population at high risk for AKI (Acute Kidney Injury). Urinary AMBP is also a marker of kidney damage in type 2 diabetes.

Apolipoprotein A-1(Apo-A1), is a major protein component of high density lipoprotein (HDL)in plasma1. The protein promotes cholesterol efflux from tissues to the liver for excretion, and it is a cofactor for lecithin cholesterolacyltransferase (LCAT) which is responsible for the formation of most plasma cholesteryl esters2. Apo-A1 was shown to assist in the improvement of predicting cardiovascular events3. The gene that encodes to Apolipoprotein A-1 is APOA1. This gene is closely linked with two other apolipoprotein genes on chromosome 114. Defects in this gene are associated with HDL deficiencies, including Tangier disease5, and with systemic non-neuropathic amyloidosis6. The protein is made up of one major isoform (pI 5.6) and two minor isoforms (pI 5.53 and 5.46). Apo-A1 shows a high content of a-helix structure6. The amphipathic regions in the -helix structure seem to be responsible for lipid binding capacity. In aqueous solution, Apo-A1 shows self-association with minor conformational change7. In addition, it has been shown that Apo-A1 is implicated in the anti-endotoxin function of HDL via interaction with lipopolysaccharide or endotoxin8.

Apolipoprotein B-48 (Apo B-48) is known to be the only specific marker of intestinal chylomicron (CM) particles. The amino acid sequence of apoB-48 represents 48% of the initial sequence of apolipoprotein B-100 (Apo B-100). Apo B-48 is synthesized only by the intestine in humans, while Apo B-100 is synthesized primarily by the liver. Therefore, Apo B-48 is a most appropriate biomarker for cardiovascular and nutritional investigation of postprandial CM metabolism.In a recent study, Apo B-48 was discovered as a novel biomarker in amyotrophic lateral sclerosis patient fibroblasts.

Apolipoprotein D is a member of the Apolipoprotein family. Unlike other lipoproteins, which are mainly produced in the liver, apolipoprotein D is mainly produced in the brain and testes. It was found to be a putative biomarker of androgen receptor function in androgen insensitivity syndrome, the most common cause of disorders of sex development. Apolipoprotein D is associated with neurological disorders and nerve injury, especially related to myelin sheath. It was shown to be elevated in a rat model of stroke, and is elevated in patients with schizophrenia, bipolar disorder, and Alzheimer′s disease.

Clusterin is a secreted glycosylated, 80-kDa disulfide-linked heterodimer of alpha and beta subunits (produced by internal cleavage). Clusterin is expressed in virtually all tissues and found in all human fluids. It is involved in numerous physiological processes important for carcinogenesis and tumor growth, including anti-apoptotic cell survival, cell cycle regulation, cell adhesion, tissue remodeling and lipid transportation. CRP is a hepatically derived pentraxin. It exists mainly as a pentamer in a cyclic structure with 5 identical 23-kD subunits in the blood. In addition to its role as an "acute phase" protein or first-line defense molecule against pathogenic organisms, baseline levels of CRP correlate positively to adverse cardiovascular events. CRP is therefore being considered a “risk factor” for cardiovascular disease. In humans, treatment with statin therapy reduces levels of both LDL-C and CRP, and concurrently there is a reduction in the number of cardiovascular events. A recent study suggests that patients with elevated basal levels of CRP are at an increased risk of diabetes, in addition to cardiovascular disease. A study of over 700 nurses showed that those in the highest quartile of trans fat consumption had blood levels of CRP that were 73% higher than those in the lowest quartile.

IL-6 is an abundant cytokine, which is crucial for leukocyte and endothelial cell activation. IL-6 is one of the major cytokines that are implicated clinically as biomarkers in ACS (Acute Coronary Syndrome). Its pathophysiological contribution to ACS is the induction of acute-phase response. Patients with ACS have increased circulating levels of IL-6 compared with those patients who have stable angina. Among patients with unstable angina, an increase in IL-6 levels that occurred 48 hours after admission compared with the admission value was associated with the combined end point of death, myocardial infarction (MI), or refractory angina. Interleukin-8 (IL-8) is a member of the CXC chemokine subfamily and is produced by blood cells and many types of tissues. The measurement of IL-8 in voided urinary samples may have utility for urine-based detection of bladder cancer. Urinary IL-8 was a strong biomarker of stress under intensive and prolonged demands, both acutely and over time. Biochem/physiol Actions
MAPK3, the first mammalian MAPK to be characterized and cloned, is commonly expressed in most tissues and is activated through the small guanosine triphosphatase Ras and sequential activation of the protein kinases Raf and MEK upon stimulation of cells with a broad range of extracellular signals. Erk was suggested to be a good marker for estradiol and tamoxifen effects. Alzheimers diseases Erk1 and Erk2 index values were inversely correlated with disease duration, suggesting maximal efficacy for early diagnosis. In addition, differential Erk1 and Erk2 phosphorylation, could have important clinical utility for providing increased certainty in the positive diagnosis of AD, particularly in the early phase of disease progression.

Biochem/physiol Actions
Pentraxin 3 (PTX3) is a secreted glycosylated protein belonging to the pentraxin superfamily1. PTX3 is rapidly produced and released by several cell types, in particular by mononuclear phagocytes, dendritic cells (DCs), fibroblasts and endothelial cells in response to primary inflammatory signals (e.g., toll-like receptor [TLR]engagement,TNF, IL-1)1. PTX3 behaves as an acute phase response protein, as the blood levels of PTX3, low in normal conditions (about 25 ng/mL in the mouse, < 2 ng/mL in humans), increase rapidly (peaking at 6–8 h after induction) and dramatically (200–800 ng/mL)during endotoxic shock, sepsis and other inflammatory and infectious conditions, correlating with the severity of the disease. Under these conditions, PTX3 is a rapid marker for primary local activation of innate immunity and inflammation26. anti-apoptotic cell survival2, cell cycle regulation3, cell adhesion4, tissue remodeling5 and lipid transportation6. PTX3 gene expression in human endothelial cells is suppressed to a greater extent by pitavastatin than the expression of 6,000 other human genes that have been examined, suggesting that PTX3 may be a novel biomarker for inflammatory cardiovascular disease8.
